*[This spending allowance] [Chronic Condition Care Assistance] [Music Therapy] [Humana Cell Phone] is a special supplemental benefit for members with specific chronic conditions who require intensive care coordination and are at a high risk for hospitalization or other adverse health outcomes. Qualifying conditions may include diabetes mellitus, cardiovascular disorders, chronic and disabling mental health conditions, chronic lung disorders, or chronic heart failure, among others. Some plans require at least two conditions. Humana is a Medicare Advantage [HMO, PPO, and PFFS] organization [and a stand-alone PDP prescription drug plan] with a Medicare contract. Humana is also a Dual Eligible Special Needs [HMO SNP, PPO SNP] plan with a Medicare contract and a contract with the state Medicaid program. Enrollment in any Humana plan depends on contract renewal.
